Please complete the following Health Questionnaire prior to the booking date, this is to enable me to ensure that the customers condition at the time of the treatment isn't going to cause any complications and I can focus on the right areas should the questionnaire highlight any areas of concern/interest.
Reflexology & Foot Massage
Please complete the following Health Questionnaire prior to the booking date if you have chosen the followings treatments: Reflexology or Foot Massage. This is to enable me to ensure that the customers condition at the time of the treatment isn't going to cause any complications and I can focus on the right areas should the questionnaire highlight any areas of concern/interest.
Aromatherapy Massage
Please complete the following Health Questionnaire prior to the booking date if you have chosen the followings treatments: Aromatherapy Massage. This is to enable me to ensure that the customers condition at the time of the treatment isn't going to cause any complications and I can focus on the right areas should the questionnaire highlight any areas of concern/interest. This form also informs me of which essential oil blend you would like to be used during the treatment.
All other treatments
Please complete the following Health Questionnaire prior to the booking date if you have chosen the followings treatments: Deep Tissue Massage, Full Body Massage, Back Massage and Head Massage. This is to enable me to ensure that the customers condition at the time of the treatment isn't going to cause any complications and I can focus on the right areas should the questionnaire highlight any areas of concern/interest.
